centennialmedcente

There are CHANGES in the requirements reเarding acceptable vaccine records

Vaccination records that meet USCIS requirements

From May 1st, 2024 to present date.

  1. Proof of Varicella History (chickenpox) is now required either by an immunity test done through our office or a letter from a physician stating the year the patient/applicant was sick with the disease. If either one of those proofs are not provided the patient/applicant will need to get the vaccination.

  2. Polio Vaccination is now required for patients/applicants ages 2 months and older. If past record is provided for this vaccine it must be three doses to meet requirements. If you do not have a record of the Polio vaccine you will need to only provide one dose. Immunity tests are not allowed for this vaccine.

  3. Tdap/Td: This vaccine is only good for 10 years and immunity tests cannot be ordered for this vaccine.

  4. MMR: The record of this vaccine must be Measles, Mumps, and Rubella. If you believe that you may have gotten the vaccine before, but don’t have record you may get an immunity test ordered through our office OR you can choose to get the vaccination.

  5. Hepatitis B: Record of this vaccine must be provided or you may get an immunity test ordered through our office OR you can choose to get the vaccination.

  6. COVID-19 vaccination needs to be the September 11, 2023-current date version for patients/applicants ages 6 months and older.
